We are open to serve you, answer questions or schedule an appointment during the following hours:

Our Mission

Malbis Parkway Pediatric Dentistry believes it is a privilege to treat children. We are committed to making a difference in children's lives by educating families and creating caring relationships in a compassionate and child-friendly atmosphere. It is our goal to create positive experiences for children in a nurturing environment with imaginative surroundings. We have created an office that is focused on children and comfortable for parents and caregivers. Every detail has been considered from the child’s point of view. Our team is passionate about what we do and we want to treat each child and family as our own.



Please respect our appointment polices as follows: We require a 24-hour cancellation notice for scheduled appointments. Advanced notice of a cancellation allows us to see other patients in need of dental care. We have reserved this time especially to care for your child. However, if the patient has failed to show for two appointments, there will be one last attempt to schedule treatment. If the third appointment is missed, the patient will become inactive in our system and will be asked to seek care elsewhere. We ask that you value our time as well as the time of others by making your appointments a top priority, as caring for your children is ours.




Parent Accompaniment

We ask that you allow our staff to accompany your child through the dental experience. We are all highly experienced in helping children overcome anxiety. Separation anxiety is not uncommon in children, so please try not to be concerned if your child exhibits some negative behavior. This is normal and will soon diminish. It is important that you trust us as your child’s practitioner, and that you feel comfortable in our office. Therefore, we invite you to come back and take a “peek” at your child during the procedure. This allows you to see how well your child is doing and to assure you they are having a good experience.



It is our office policy that a child’s legal guardian accompanies them to their first visit. Thereafter, if you are unable to bring your child, you must sign our Consent To Accompany form giving permission for someone other than you to be present for the treatment. We also ask that you be available by phone so that we may contact you if there are any changes in treatment.



Payments / Insurance

Our team would like to make your visit a positive experience and are happy to answer any questions that you might have regarding your child’s appointment. Our policy requires copays, deductibles, and non covered charges be paid at the time of service. To assist with your payment, our office accepts cash, checks, Visa, MasterCard, Discover, American Express, and CareCredit. Personal checks are accepted with proper identification (DL or Photo ID). A $30 overdraft charge will be posted to your account for each insufficient check.


CareCredit is a convenient, low minimum monthly payment plan with no interest, designed for your entire family to pay for healthcare not covered by insurance. When your bill is unpaid, a collection agency may be chosen to manage delinquent accounts. If your account is placed with a collection agency, you will be responsible for all costs of collection.


Insurance - We are providers for Blue Cross/Blue Shield of Alabama,  AL Medicaid, Metlife, Cigna, Delta Dental, Southland and ALL KIDS. We also accept other insurances, and can file your claim for your convenience. We do not file secondary insurance claims, however, we will be happy to assist you through the process. If you are a member of a Dental Insurance Plan and have chosen us as a provider of your child’s care, it is your responsibility to:

  • Provide us with information relative to your claim, including insurance card, ID number/group number, employer, name of insured, date of birth, address, and Social Security number. This information is requested on the Patient Information Form, which we ask that you complete during your initial visit.
  • Pay your deductible and/or co-pay at the time of service.
  • Pay for services not covered by your insurance carrier upon request. Insurance claims for your carriers are filed as a courtesy to you.
